Home Health

Leaving the hospital doesn't mean an end to the healing process. You or a loved one may benefit from Home Health Care: hospital-quality care administered in the home.  Most often, this type of care is needed following a hospital stay, same-day surgery, or birth of a baby, but may also be prescribed to monitor changes after a doctor's visit, for example, a change in medications that the doctor wants to monitor.

The benefits of Home Health care may include a faster recovery because patients are in familiar surroundings with support of family or friends acting as caregivers.  Patients have a goal of an independent lifestyle.

While a patient at Titusville Area Hospital, Home Health staff can become familiar with a patient's condition and care plan in order to provide at-home follow-up care accordingly.

Services are also available to patients discharged from other area hospitals or nursing homes.

  • Occupational Therapist
  • Speech Therapist
  • Respiratory Therapist
  • Home Health Aide Service
  • Dietician
  • Social Services Coordinator

Home Health can provide:

  • A personal emergency response system
  • Wound care
  • Diabetic teaching
  • Ostomy teaching
  • Cardiac surgery care
  • Patient teaching related to Congestive Heart Failure (CHF)
  • Patient teaching related to Chronic Obstructive Lung Disease (COPD)
  • IV therapy
  • Catheter care and instructions
  • Referrals to other home support programs through County Area Agency on Aging

Eligibility

To be eligible for Titusville Area Hospital Home Health Care, patients must:

  • Be referred by a licensed physician
  • Have a medical need that requires skilled  care
  • Be home-bound at time of service
  • Reside in Crawford, Forest, Venango or Warren counties

Services are provided to eligible patients without regard to race, color, gender, sexual orientation, religious creed, ancestry, age, national origin, handicap, disability or ability to pay.

Hours

The Home Health office is open 8:00 a.m. to 4:30 p.m. Monday through Friday.

Home care visits are available seven days a week, and emergency coverage is available 24-hours a day by calling the Titusville Area Hospital Emergency Department at 814-827-1851.

Payment

We accept Medicare and Medicaid as well as Commercial Insurance Plans. Some insurance plans require pre-authorization and may be subject to co-pays and deductibles.
